We accept referrals from GPs, specialists, and hospital discharge teams. DVA case managers may also refer directly. DVA-funded services are subject to DVA approval and clinical assessment.

All patients receiving care from SWAST CARE are entitled to the rights set out in the Australian Charter of Healthcare Rights, including the right to:
- Access safe and high-quality care
- Be treated with dignity and respect
- Be informed about your care and treatment
- Participate in decisions about your care
- Have your privacy protected
- Provide feedback or make a complaint
Complaints & Feedback
We take all feedback seriously. If you are unhappy with any aspect of your care, you have the right to raise a concern at any time — without it affecting your care.
Step 1 — Contact Us Directly
Speak with your nurse or contact our office at [email protected]. We aim to acknowledge all concerns within 2 business days and respond within 10 business days.

If unresolved, you may contact the Aged Care Quality and Safety Commission on 1800 951 822 or the DVA Client Feedback Line on 1800 838 372.

Interpreter & Language Services
If you or your patient requires an interpreter, SWAST CARE can arrange access to the Translating and Interpreting Service (TIS National) at no cost to DVA-funded patients.

Independent Advocacy
You have the right to have an advocate support you at any time — including when making a complaint, attending care meetings, or making decisions about your care. Advocacy services are free and independent.

Consent & Advance Care Planning
SWAST CARE obtains informed consent before commencing any care. You have the right to refuse or withdraw consent at any time without it affecting your access to other services.
Informed Consent
Our nurses will explain all proposed care and obtain your consent before commencing treatment. You may ask questions at any time.
